2011 TSM Award Nomination Form

THE CENTER FOR TSM IS CURRENTLY SEEKING NOMINATIONS FOR ANY OF THE FIVE TSM AWARDS:

The Dr. Gene Wells Ray Telecommunications Systems Management
Lifetime Achievement Award
This award is presented to an individual that has exhibited a significant, sustained contribution to TSM over a career.

The Dick Anderson Telecommunications Systems Management
Leadership Award
This award is presented to an individual that has made a significant contribution to TSM by their leadership, technical and/or management skills.

The John Williams Telecommunications Systems Management
Entrepreneurship Award
This award is presented to an individual or company that has started a TSM related company or significantly expanded an operation in Kentucky or the MSU service area.

Telecommunications Systems Management
Distinguished Alumni Award
This award is presented to a TSM graduate that has brought credit to themself and Murray State University. (Criteria: 5+ years post-degree, Bachelor's or Master's degree in TSM, made a significant contributiton to the field of TSM or MSU.

Telecommunications Systems Management
Faculty/Staff Excellence Award
This award is presented to a faculty or staff member for excellence in teaching, research, and/or community service. (Criteria: Current or former faculty or staff member of the TSM program at MSU; significant event in teaching, research, and/or community service in a single year; significant performance in teaching, research, and/or community service over a period of years.)

NOMINATIONS
Nominations will be accepted through March 18, 2011. Submissions received after the deadline will not be considered. Only one nomination per person will be accepted. Self nominations will also be accepted. Incomplete applications will not be judged.

JUDGING
Selection of finalists will be made by a selection committee consisting of MSU Administration, faculty and telecom professionals. Decisions will be based on the characteristics listed above. Questions can be directed to Dr. James Gantt at james.gantt@murraystate.edu.
