In general, the questions below represent the most frequently requested information:


Question 1: What should a person do if they believe that they have experienced harassment or discrimination which violates the university's non-discrimination statement? 
Answer: A member of the University's community who believes themselves to be a victim of harassment and/or discrimination is encouraged to report the information to the Office of Institutional Diversity, Equity and Access (IDEA).

Question 2: What should a person do if they witness or learn about an incident that may be considered harassment or discrimination? 
Answer: All members of the University's community should report any information they learn about discriminatory, harassment, or sexual misconduct is against a faculty or staff employee of the University to the Office of Institutional Diversity, Equity and Access (IDEA), 103 Wells Hall, 270-809-3155
If the discriminatory, harassment or sexual misconduct behavior is against a student of the University, please report any information learned to the Deputy Title IX Coordinator and Associate Vice President of Student Affairs and Deputy Title IX Coordinator (425 Wells Hall, 270-809-6832).

Question 3: Where can I find information on how to write out my account of what occurred to file an official complaint? 
Answer: Forms may not provide enough information about the concern.  To that end, our complaint process involves a narrative or written account detailing what occurred.  Starter questions for event documentation or official complaint preparation are available on the website: www.murraystate.edu/idea and choose "Discrimination Grievances" from the left side of the screen.  A PDF file version of these starter questions is available here.

Generally speaking, when preparing your written complaint, you may include any information you feel would be appropriate to filing the complaint.  Please provide an account of the incident(s) that you feel demonstrates discrimination and/or harassment.  Below you should find general starter questions and tips to help you begin documenting your account:
    1.  Prepare your account/documentation which will answer the following questions:
         A. What occurred and was done specifically that leads you to believe you have been harassed/discriminated against?
         B.  When and where did the incident(s) occur?
         C. Who were the individuals involved?
         D.  Who witnessed the incident?
         E.  Has the incident occurred more than once?  If yes, when, where and how long?
         F. Identify any evidence that this incident was motivated by the complainant's/victim's (Race, gender, religion, color, national origin, age, gender, sexual orientation, veteran status, or disability).
         G.  Have you shown or told the person that the behavior is unwelcome?
         H. Relief Demanded: What would you like to see happen (what action are you seeking to resolve the matter?)?
         I. Whom have you spoken to regarding this matter? When?
         J.  What is the role of the accused individual on campus: faculty? staff? student? visitor? vendor?
         K.  What is the name the accused individual(s) on campus?
         L. Please provide today's date, your printed name and a signature. (You will sign at the end of your document)  
         M. Additional notes: You know your complaint better than anyone else. Give the IDEA all of the details and answer all questions as fully as you can. Names, dates, places, and details of what happened should be as accurate as possible. Supporting documents, such as email messages, payroll slips or performance documentation, etc. can help to support allegations. If witnesses were present, it’s important to give full names, what they will know and how the IDEA can contact them.

    2. Additionally, you will want to:
        A.  Check your document for grammatical or spelling errors to promote readability.
        B.  Keep the document in a safe location; not lying around.
        C.  Make a list of whom you spoke with about the incident(s).
        D.  Avoid talking to others about the incident(s).
        E.  Avoid attempts to investigate the matter.
        F.  Call the Office of Institutional Diversity, Equity and Access at 270-809-3155 to schedule an appointment to discuss the next steps in the process.

   3. Contact information:
       A. Identify who the complaint against (Name of the individual(s)).
       B. Identify your name and contact information (address, phone number, and email address)

Question 4: How long do I have to file a grievance based on alleged discrimination?
Answer: A grievance based on alleged discrimination must be filed in writing within one-hundred-eighty (180) calendar days from the date of the alleged discriminatory act.

Question 5: What if I am not sure if what has happened to me is really discrimination?
Answer: Call the Office of Institutional Diversity, Equity and Access (IDEA) to assist you in making your determination, and field questions about the grievance procedures/process. Generally speaking, the grievance procedures are to address allegations of discrimination based on the institution's non-discrimination statement.
