this form must be completed by the applicant and presented to the Division of Driver Licensing or a Division

of Driver Licensing field office.

in this application are true and correct.

Along with this application, applicant must present at least one of the following original documents:

Passport with valid I-94 card

Employment Authorization Card (I-68B or I-766)

I-94 issued by INS with a stamped picture.

Applicant must also present the following documentation:

Social Security Card, Tax Identification Number, or Ineligibility Letter from the Social Security Office

Official proof of residency (proof of address within last 60 days) i.e., utility bill, lease agreement,

bank statement, credit card statement or postmarked envelope.

Students who are F1 status will need their (I-20) and proof of full time enrollment from the Registrar's

office.
