in this application are true and correct.

Along with this application, applicant must present at least one of the following original documents:

Passport with valid I-94 card

Employment Authorization Card (I-68B or I-766)

I-94 issued by INS with a stamped picture.

Social Security Card, Tax Identification Number, or Ineligibility Letter from the Social Security Office

Official proof of residency (proof of address within last 60 days) i.e., utility bill, lease agreement,

bank statement, credit card statement or postmarked envelope.
