The following forms are for Official MSU Student Organization use only. 

1. Print the form using the links below.
2. Complete the form with very specific parameters
3. Include any letters or documentation that will be given to students
4. Signature is required from a faculty/staff sponsor of the student organization
5. Take completed form & documentation to Jim Baurer ( 3rd Floor Curris Center Office) for approval and additional guidelines
6. Please allow 1 week to process the request. (Most requests will be processed within 2-3 days.)
