        Please complete this scale to get an idea of your overall health and wellness.  At the end of the scale you will find a scoring chart.  After scoring the scale, If you have a score below 70, you might want to explore alternatives to increase your health and wellness.  If a person stays below 70 points for an extended period of time   they have a high chance of becoming ill.  Contact Corky if you have any questions/concerns about your "General Well Being" scale or your results.

The purpose of this class is to promote a healthy lifestyle.  It is designed to help individuals learn more about their own health & wellness through the acquisition of knowledge, evaluation of attitudes & behaviors, and actual participation.  Participants will monitor and assess their own health and wellness activities and participate in various workshops and three mandatory meetings on issues that promote lifelong fitness.

You will not get or need a student ID if you are faculty/staff.  The Wellness Center card reader will recognize your ID (for Spring 2008 only) once you are officially enrolled in this class.

1.  HEA 189 is offered on a pass/fail basis.  Please carefully review the syllabus for course requirements.  This course requires:

  • Attendance at the Health Fair  in Carr Health

  • Completion of a diary documenting a minimum of three 45-minute workout sessions per week.

  • See the course syllabus for more information

2.  The tuition waiver must be completed as soon as possible.  You will not be able to use your faculty/staff ID in the Wellness Center for Spring 2008 until you are officially enrolled.  If you anticipate using all of your waivers for other courses during the fiscal year, this HEA189 Spring 2008 course will not count against the current limit.  Call Human Resources at 809-2146 for more information about the waiver, or tell them of your intent when you deliver your waiver form:  Tuition Waiver Form(.pdf)

3.  If you are not currently enrolled or eligible to schedule and the only course you want is HEA 189, use this streamlined application for admission:  Application Form(.pdf) If you are seeking admission for HEA 189 and/or other classes, use the standard form.  Or, if you are a faculty or staff member & already eligible to schedule you can email Kristina.Jackson@murraystate.edu to be added to the course.
