College Housing Application
You may download and print this application and complete the application.
The appliction must be either mailed or delivered to the Housing Office
with a $150.00 deposit check or money order to become an active application.
This application includes information about James Richmond Hall (New Richmond
College) that is due to open in Fall 2009.

Courts Apartment Application
You may download and print this application and complete the application
on either page 3 or 4 depending on your circumstances. You may return
it to the housing office or mail it to the address below with a $150.00
deposit check or money order.
Summer
School 2009 Application
You may download and print this
application for Summer School 2009 Housing. If you have not applied for
housing before and have a deposit on file a $150 deposit may be required.
Miscellaneous Housing Forms
Request for Reasonable Accommodation for University Housing
You may download and print this application for a request for reasonable accomodation if you feel that a special circumstance or situation such as a disability. This form includes instructions on the form for the proper completion and submission of the form.
Faculty
and Staff Discount Letter and Form
You may download and print out this form for faculty and staff members
of Murray State University to use to obtain a $100.00 discount on your
son/daughter's residential college room for the upcoming semester. You
may send the form back in the mail or fax it at the telephone number below.
Federal
I-9 Form
Every student who is hired
by the Housing Office is required to complete Section 1 this form within
2 business days of being offered the position and return it to the housing
office with the appropriate documents listed on page 4 of the document.
Failure to complete this form within the specific time period may result
in the employment offer becoming void and null due to failure to cooperate
with proper payroll proceedure.

Housing Exemption Forms
On-Campus
Housing Exemption Application
You may download and print this application if you are a freshman or sophomore
and commute daily from parents home. This form is also used if you are
requesting an exemption for other reasons (see the Housing Exemptions
page for more information). The Housing Exemption form must be submitted
along with the Notary form and the Housing Exemption Agreement of Understanding
form. The Notary Form must be signed by the parent and a notary. You may
either return these forms to the housing office or mail them to the office.

Exemption Agreement of Understanding
You may download and print this
form that must be submitted with all requests for On-Campus Housing Exemption
Applications. It must be completed by the student requesting the exemption.
Exemption and Notary Form
The Exemption Notary Form can be filled out by your parents and Notary Public to verify that you will be commuting from your permanent address. This form must be submitted with the Housing Exemption form to the Housing Office.
