You may download and print this application for a request for reasonable accomodation if you feel that a special circumstance or situation such as a disability. This form includes instructions on the form for the proper completion and submission of the form.
Faculty and Staff Discount Letter and Form
You may download and print out this form for faculty and staff members of Murray State University to use to obtain a $100.00 discount on your son/daughter's residential college room for the upcoming semester. You may send the form back in the mail or fax it at the telephone number below.

Housing Exemption Forms
Housing Exemption Request Form
You may download and print this application if you are a freshman or sophomore and commute daily from parents home. The Housing Exemption form must be submitted along with the Notary form. This form must be signed by the parent and a notary. You may either return these forms to the housing office or mail it. After you submit these forms you will need to meet with a member of the Housing Exemption Board to finish the process. For more information, be sure to check the Housing Exemptions page.
Exemption and Notary Form
The Exemption Notary Form can be filled out by your parents and Notary Public to verify that you will be commuting from your permanent address. This form must be submitted with the Housing Exemption form to the Housing Office.
